This guide will tell you what to expect and what you’ll need

before calling Montgomery County’s Office of Aging Services.

What You’ll Need to Tell Them

Before you call, make sure you know:

• Your full name

• Date of birth

• Home address

• Phone number

• Your Social Security number

• Name and phone number of an emergency contact

• Name of your doctor (if you have one)

• Info about your health, like:

• - Any medical problems or conditions

• - If you use a walker, cane, or wheelchair

• - If it’s hard for you to cook or go shopping

• - If you live alone or have help
